Does he exist in the town? If so, you can use "testingcheatsenabled true" as a cheat, shift+click on him, and choose add to family. I've heard of this happening to infants, but never adult sims.

I think it'd be better if ghosts could become invisible at will, maybe with a cooldown, but have the default stay visible. It would limit them so much, to be changed the way you suggest.
Do you have an aura on, maybe? Click her, choose (I believe) Aura, and if the option to stop is there, you have your solution. Or try resetting her. That's my first response when something weird happens. XD
There's no need to fight. Obviously you never got or completed the opportunity chain, which I'm assuming is the progression of certain opportunities--like the ones in cooking that lead to cheese and egg plants, then meat plants, then iirc omni plants. See, opportunities don't stand alone--think of them as levels, or steps…
I'd like to take issue with your 80% figure. From the forum here, it seems like most people have one issue or another. I'd think it's more like 20% of players, if that, have no issues whatsoever.
