This is what happens: If a sim dies in Sulani, their tombstone or urn pops up but there is no Grim. But if a sim dies in any other world, Grim does appear. I don’t know if this also happens with NPC sims. So far I have only tested with played sims and only on the home lot (which has usually been Pier Perfection).
This does not seem to be a problem in all saves. I have tested it in 6 of my saves: in three of them, Grim does appear in Sulani and in three of them he does not. I have also tried it in a new save and in that new save Grim did appear in Sulani. Interestingly, the saves in which Grim does not turn up in Sulani are my more long running ones. One of them has gone on for 10 generations and another for 8 generations.
By the way, this is without mods. But even when I had mods installed I only had MCCC and a vampire powers mod by littlemssam and I have never had CC.
The saves in which Grim does turn up in Sulani are only 2 generations or 1 generation.
So here is my question:
Please can especially those with long running or multi generational saves kill an active sim on their home lot (preferably Pier Perfection) in Sulani and see if Grim turns up?

Grim Reaper didn't show up when my sim died on dry land.

Hmm I would have said the same but I have found the following with three of my saves:
Save 1 (begun June 2018): Grim doesn’t turn up
Save 2 (begun September 2018): Grim does turn up
Save 3 (begun September 2018): Grim does turn up
Save 4 (begun October 2019): Grim doesn’t turn up
So, Saves 2 and 3 are actually older than save 4 and yet Grim turns up in saves 2 and 3 but not in save 4.
The common denominator in my game seems to be that the saves in which Grim doesn’t turn up have been played for multiple generations. For example, save 1 has been played for 11 generations and save 4 for 7 or 8 generations. It could also be happening in saves which have been played a lot. I haven’t played saves 2 or 3 very much, but I have played saves 1 and 4 a lot.

Ohh... If you have a family tree that you want to save and there are elders around I really recommend using flower arrangements. It is done quickly and then you just release the spirit and you're done. It is amazing! In the olden days I had pools with red water and it just took forever for my poor little elders to finally croak. Now all my npc-families have intact family-trees and I like that. As a rotational player with many families this has been very useful.

Hm... some households are on the 3:rd generations but then Mila Munch is about 205 days old and an elder now. I switch aging on and off in my saves so how much time that has passed can be difficult to keep track off. Don't know how helpful that was

Or might be true. I dunno. In my oldest save grim still shows up in Sulani, but he also said something along "Stop killing sims, I don't have all day to reap the dead" when my sim accidentally killed a rival (due to her enabled autonomy for extreme violence)
on another death by natural cause (not because of a mod) he also showed up, but he appeared so briefly my dead sim's relatives didn't have enough time to plead. He just popped out, swung the scythe, and disappeared. No checking his tablets or dilly dallying.
