I've had similar problems like that before. They were on the verge of dying, and they just didn't. They're not teens are they? Are they young adults? If you are looking to kill sims you can try mods? There's a few good ones, but some are bugged. Extreme Violence works.

To be honest, I don't think this is something you'll get in TS4. This really sounds like something that would happen in a spin off. Muuuch like TSM.
