More instruments, can add up to 5 traits for young adults +, 4 traits for teens, 3 traits for children, 2 traits for toddlers, more careers, less stuff packs, in-depth expansions with more immersion/content, random events happening at the home, burglars?... lots of things... The Sims Studio needs to look back at the previous iterations to understand how immersed players felt with the random events.
Spiral Stairs, L-Stairs..in general, better stair flexibility and the ability to place stairs at a diagonal. The ability to have differentiating foundation heights Colors that match, from different game packs, in Build mode (Nothing more frustrating than having the same color wood all be different colors in a build.) The ability to use our own screenshots for the Gallery. The ability to build on half and quarter squares. Something other than a 45 degree angle for walls. A way to shut off the snap to for windows and doors and a number of other things.
Theme parks; Roller coasters (we have first-person view now); Water slides; Ferris Wheels with make-out options; Fun houses; Carnival games (that kids can also play and win prizes, like giant bears, goldfish, and such).
I would love some more animations with babies, if we could move them without cheats to a rocking chair or hold them as our sims did things around the house. Pre teens would be great too, its an odd jump from child-teen, preteens would be fun!
I can never get enough of things my sims can do for amusement, so carnivals or amusement themed park with rides would be fun. A carnival theme could go with a farming pack. A casino could be fun. Sims could get rich or break the bank.
What I would love is to be able to create hotels and apartments and have them function as such.
I think a height slider for adult sims would be cool.
I want that Punk/Goth/Rocker Stuff Pack @Movotti mentioned in a diff thread.
I like @aprilrose's idea too: I want sims to automatically know their relatives and for the invite household over button to come back.
I'd also like a sectional or an L-shaped sofa.

When you have a married couple and one of them asks the other "What do you think of my friend?' and the other says "I am SO in love with them", if they don't have the serial romantic trait the spouse gets enraged with jealousy.

Definitely split-level staircases. Plus, when you visit Strangerville and move the camera around to see the world, you can see an L-shaped building (supposedly a motel) and it clearly DOES have a split-level staircase. So why the game doesn't have one yet is beyond belief. They can clearly be done.
Also, there needs to be more kissing animations. When you have two people in love kissing, especially a married couple, you'd expect to go on a bit longer than a couple of seconds.
Sim interests/likes/dislikes/turn-on/turn-offs/favorite similar to TS2's systems (that players may set in CAS)
A dedicated wants and fears panel similar to TS2 - the system added a layer of interest to the game because it seemed as if the player could glimpse into the sim's inner thoughts (i.e. what the sim desired to achieve and what he/she wanted to avoid)
Sim personality (similar to what was available in TS2 and, later, added to TS3 as well) so sims with greater distinctive characters may be created
"Fertilize all" option for gardening

(I've had my community lots glitch out when I've built restaurants (using Dine Out assets) in other community lots like CL's Art Centers (NPCs sims, without uniforms, actually showed up to cook and wait on sims but my sim couple that went to the ART CENTER for a date couldn't fulfill a single requirement on their date because the game thought they were at a restaurant? The lot was set as an Art Center that I built a bar/restaurant on the ground floor of )

More incentive to play elder sims - e.g. food elders make/talks they give/hugs they give give unique buffs, they can calm a fussy baby better (or even worse, depending on their traits - say, if the young one is afraid of them), unique recipes will unlock for sims at this stage of life (e.g. "old family recipe" that offers a buff such as uniquely related to the 1) making and 2) eating of the dish), different types of stories unlock at this stage of life, etc...
For TS4 not to be mainly a young adult life simulator - other stages of life need more incentive to play them. In game, what differentiates a young adult sim and an adult sim beside a few physical changes?--nothing, in game as far as I can well. For example, adult sims seem to be able to workout just as hard for just as long w/o trouble in comparison to teens and YA which in RL, would most likely be unlikely.
Befriend-able and adoptable llamas, mischievous or sweet freezerbunnie creatures that are similar to gnomes (it's already in the game, make a freezerbunny mimic asset, please), and llamacorns

Cats & Dogs: pets not getting sick often especially if they are well taken care and not neglected of & ability for children to take dogs out for walks (raises responsibility gains if Parenthood GP is installed)
Get Together: pet care and pet activities added to Club Activities so making a dog care/dog walking service is possible
Get Famous: relatives will no longer fawn over celebrities; ability for celebrity's good friend or relative to introduce celebrity to another so that that sim won't be star-stuck/making an excited fuss over the celeb; for celebs not to be so disruptive to gameplay--one can't even sell to them with GTW installed because of their celebrity status
