The two obvious ones of course... MCCC and UI Extensions (without which, my disk space would be freed up, unless a miracle happens at EA and they bring out a pack that... *SHOCK* fixes the base game and all pack bugs and issues... *sigh* yes, I know, but we can dream can't we?!?)
I'm rather partial to any/virtually all of Coolspear's mods, Zerbu's Turbo Careers and Neia's Career mods - all excellent mod artistes and long may they continue.
I saw you mentioned realistic mods. Honestly, Period mods. Is it weird that I wish the same pain upon my sims that I have to deal with every month? It probably is.

Idk, there are just some Sims that I like to be secure and unharmed.
I haven't used CC or Mods for Sims 4 yet, so I apologize if this is a) already out there, b) impossible, or c) I don't know what I'm talking about (which is likely), but one mod that I would break down and get despite my aversion to them is some sort of inventory sorting mod. Like, when it's time to fertilize, or pick bait, or even find something in a busy family or individual inventory. The ability to sort by type or group items together by more categories would be indispensable. I hate trying to find that fish among a ton of crops when looking to fertilize, and the way things are sorted in the bait menu makes no sense to me.
I use no mods or CC and I repair my game after every time I play.
