Here's a list of things I use. These items should work for you too, assuming you have a PC with a discrete graphics card.
Capture software, I use Dxtory, there's also Fraps, Bandicam, etc
Audio software I use Audacity, its free
A microphone a lot of people start with a Blue Snowball, I use a Rode NT-USB on a scissor arm attached to my desk, because I'm clumsy and kept bumping into my mic/desk.
A pop filter to soften any plosives
A set of headphones to listen to your game audio and your voice while you record.
Video Editing software. I use Sony Vegas Pro 12, other choices are Sony Movie Studio, Adobe Premiere Pro/Elements, etc.
Software for making thumbnails, I use Gimp and Inkscape, you could also use Adobe Illustrator, Photoshop, Photoshop Elements, etc.
Optional: an extra hard drive for recording your game footage on
