It depends what site you're downloading from, and what type of file it is.
If you download a sims3pack file (that looks like the sims 3 icon) it needs to go in your sims 3 downloads folder (Documents > Electronic Arts > The Sims 3 > Downloads) and it will show up in the 'downloads' tab in the game launcher for you to select and install
If you've downloaded from a site that doesn't offer .sims3pack downloads, and the files are in .package, .rar, .zip or .7z format, you'll need to set up some framework in your sims 3 folders before you can install the files.
