frozen action; never completes

At first it was just my painter sim-- I'd ask her to go and finish a painting, it would be finished, but she would keep on painting... I'd have to cancel the action and then it would complete and she could move on to the next in cue.
More recently and more annoyingly however, several of my sims will finish eating and then fail to move on to the next task! I have to click on the task to cancel, then they will wash their dishes, and move on to the shower or bed or whatever next.
Painting I could just deal with, but all my sims eat twice a day, it's getting to be more of a hassle to micromanage them, and tell them to stop eating and move on!
What's causing this/ how to mend?

Also, does any of your Sims has a "Perfectionist" trait? If so, those Sims takes time to complete their task.
