Years struggling with this sound and video issue, and still no answers

In 2020, the sound stopped working properly for my game, and I started having this old bug:

I have been through the ringer trying to resolve this issue for two years, and still no answers. I've done everything under the sun you can possibly think of, and the problem has followed me across two operating systems, two different graphics cards and three different setups.
I keep hearing over and over again that I just need to update my drivers, but this can't be the issue. Not only did I update my drivers a million times and back, I now have a dedicated sound driver. I have tweaked the Windows sound settings a million times over.
My suspicion is that it has something to do with audio and video codecs, but I have not turned up any leads.
Is there anyone who might have some insight into this issue? Thanks.
