Well, HalloweenChild explained the first part.
The second part being the toddler....
Its a glitch that happens from time to time when a sim gets stuck between age states. Mostly happens when tinkering around in CAS I believe. At least, it does generally for me. But a good old 'resetsim' generally fixes the issue and turns them back to their toddler self.

Erm...You do know that EA owns the Sims 3 and not Maxis, right?
