They presumably mean your "custom" content - do you have any custom content (third party content) or mods (hacks) installed? If you've never used any such content then you can go ahead and patch. If you do have such content then it is always best to remove it before updating your game.

Do you mean the small images in a folder called "Featured Items"? They are safe to delete but will regenerate every time you start the game unless you make that folder Read Only. They are there because of the in-game Store but they do pile up in their hundreds or thousands and it is nice to be rid of them.
