When something you download from the exchange is marked as having "Premium Content", it means that it uses items, patterns, or clothing available in the Sims 3 Store, which you have to own yourself first in order for the user content created with those items to be installed. Hope that helps. :D

You can upload Families (a single Sim alone on a lot counts as a "family", btw) as well, which when you install them, will appear in the Sim Bin. You can then place them as a neighbor to your Sim while in Edit Town mode, if you like. :D

Actually, what you need to do is go to the page for the Sim that you've uploaded. From there, next to the portrait, there should be a button somewhere saying "use this Sim as my avatar". Just hit that, and it should switch over. Hope that helps. :D
