Ghosts have to spawn from their urn, so it has to be actually sitting on the lot somewhere. If their urn is deleted/disappeared or even just in someone's inventory, then it can't spawn its ghost. As long as you have an urn on your lot, and its resident dead sim hasn't been sent to the netherworld, a ghost can show up. If a sim dies and you aren't currently controlling the household (even if you have played it before), there isn't an urn, so you can only get ghosts out of households you play.
