Find a townie whose face you like (even if its just a feature or two), save then to the bin, and tweak from there. It can be a lot easier than trying to create a unique face from scratch.

My vote is kind of both? When building walls and wallpapering, I have them on. When putting in flooring and decorating with furniture, I have them off. Though I will sometimes turn then on briefly to see if a sim will have room to route properly past something.

None. I guess you could make an argument that I skip Elder, since I jump from save to save so often that my sims never really get there, but if they do I dont just shuttle them off to the side. Baby is one that I want to skip, but the challenge of them makes my games feel more real.
Even further, you can place the seasonal marker on any commercial lot. So the whole town can change season by season, though that may cause quite a bit of performance drag.
I recently had a similar happening, my sim got scorched but the ground beside her lit up in flames. I thought it might have been a glitch, but maybe not!
