My number one thing would be Story Progression. After that I want Sims to behave better, like, if they come visit, BE social, if you watch a movie, SIT and watch it... focus.
I would like if the traits played more into a sim's personality like how they did in 3, and perhaps make relationships more dynamic/realistic, for example: Realistic in that If my sim is enemies with someone it doesn't make sense if suddenly they're being nice and laughing together when their relationship panel may show a full red bar and more Dynamic in that if im playing a household of people their relationships fall down and build up mostly through their own interaction with each-other so that I dont have to put in effort to make them not just acquaintances the entire time they live together and what-not.
Another thing would be some sort of inheritance system like there was back in the Sims 2 where when an elder sim died their relatives would receive money in varying amounts from them.
^ I agree on the relationships, I'm also dying for supernatural things to enter as well. Love vampires but i wouldnt have minded waiting if they added a few more spooky types with the pack.
I'd really like consequences back, I want a chance card that has the ability to straight up fire my muse, like in sims 2. I'd also really like to have career whims back. I'd like traits to influence my sims more and when I have my sim cheat on their significant other I want them to care I want them to be angry. I want them to have an anger that can't be fixed by a pretty picture.

Besides the wacky, odd things that made TS2/3 unique, I really, really miss being able to invite a household over. If I'm having a wedding, I want to be able to invite Aunt Judy, Uncle Bill and my cousins all at once.
I also want extended families to automatically be registered to babies and not have to invite them over and have the toddler go up to each person individually in order to meet them.
I want my sims last names to change after they get married - or at least have a way of doing this outside of CAS. It would be great if there was a pop up or something that gave an option of which name to keep or whether to hyphenate the names.
I also wish sims would stop sitting down at the table when they put a book down. I don't know if this is a glitch as I don't think sims used to do this. Previously they'd either put then in their inventory, back in the bookcase or just down on the nearest surface. Now my sims put the book on the table, sit down on the chair, and then get up to do the next action - very frustrating!!
I would like more everyday details, and specifically such details that would make each home a tad different.
One such detail: Never understood why all homes have the same boring white plates? Wouldn't take much to allow each house to have their chosen color on plates and coffee mugs, solid colors would do, but additional discrete patterns would be nice. Same for table cloths and kitchen towels.
Also would like individual accounts in a household. Bills and buying could still go from a common account, but it would allow us to decide who would transfer the needed amount to the common account in order to buy. Most important, all income would go to the sim who actually earned them Add bank/invest/insurance products to this
Always wanted sims to have small random flaws, like skin marks or beautispots that we do not decide where shall be. Also wish the teeth variation would be genetic, that you could fix teeth, but would still pas on "charming" teetjh to next generation.
Wish there was some fertility parameter to each sim, making it easy or hard to have kids. Not dramatic changes, just enough to make some need to try longer.
I really really wish elders would get weaker at some stage. I mean, there should be a difference being 70 or 100. A simple details as the TS3 walking cane would be enough, but this could be done so much with. In my game not a single sim died in bed...

If you have them only do cardio, then they don't build muscle.
What's cardio in the game? What do I make them do in the game to do the fast walk?
I have one sim only using the tread mill and she has muscles on her legs. She's only level 5 in fitness. I had to get her weight down after she eat a meal in the jungle.
If you have a TV or access to a TV, there is an option to do 2 different workout routines.

- inviting friends of the entire household over for a planned event, not just the host's friends.
- placing fences on the very edge of the lot (we had the technology in sims 2)
- Putting together a will for splitting money when a sim dies. (I feel like this should be more than possible with how advanced technology is)
- going on a stroll with babies, taking babies to weddings, who just leaves their baby every time they go to a public event???
- Dancing with kids, like the kids standing on the parents feet as they slow dance come on that was so cute!
- pretty much more in dept animations in general.
