The top two for me are university and more jobs, luckily I would assume a pack would bring both. Otherwise bug fixes & smarter sims would be next on my list.
I chose university although having smarter sims would be revolutionary. I hope one day we can get to that point! Being surprised at sim capabilities would be super cool

Agreed! I dont see the point in adding more expansions and what not when the current ones are already full of glitches, or as you said not been explored by many others.
I did other cause I want to see multiple updates, as in 1) Movible babies like in sims 2 and 3 2)pre-teens like there can be random buffs that say "Growing pains" (uncomfortable) or "ewww greasy hair!" (uncomfortable) 3)for the pre-teens slowly through there life-stage for girls they're chests slowly expand. 4)romance for the pre-teens 5)a more open world like in the sims 3 (or 2 I can't remember) where you have to ride a bike, or drive a car 6)MORE PETS I am talking horses, lizards, medium dogs (not small or large), Extra-large dogs, Turtles, Frogs, And more! 7) more realistic babies and a life stage in-between toddler and child (perhaps preschooler)
Romance is what I picked. Romance is really lacking in TS4. That said however my 2nd choice would be other which would include a Beach Vacation Destination,More Pool Items,A Carnival Cruise Pack,Amusement Parks and or Waterparks and last but not least cars.
Open world is def. what is missing for me in S4, however, it probably shouldn't be listed because it can't be fixed in S4 from what I've read.
Farming EP would be my #1 a co-op to buy/sell livestock, seeds, equipment, and feed. More 64x64 lots, many animals that sims can breed that don't add to house count, but just roam in pens/fences that we make. Happy Acres has animals that roam all over the lot although all you can do there is feed and get supplies. Farm equipment & tractors(old and new), 🐸🐸🐸🐸 & hand plows for poor/off grid existance, and ability to buy hay or bale it(round & square). Fences-electric, barbwire, old stick/rickety, & plank fence. Farmer career. antiques. Premade barn & shed or ability to build for those who like that. Emphasis on farming/equipment/harvestables/animals and not bloated with CAS/build buy. We can always get hair/clothes, but we can't always get more farming stuff.
More jobs is my #2 I really want housebuilding, interior decorator, stylist, babysitter, nanny(we can be the nanny), farmer, & small jobs(grocery store, pizza place, factory).
Grim interactions, graveyard, wills, funeral services, pick out casket, tombstones(old/mossy/fancy/large/small/crumbling/leaning)& urns(cheap/fancy/goth). More goth stuff. All could be in a Life & Death EP with Angels since we never had them in S3.
University/high school/grade school/pre-school/daycare could all be in a business section type of hood like the stores part of Get to Work. They could be open and ability to teach depending on what kind of college is completed. But, I would only want this after Farming, more jobs, and goth/grim stuff.
There are several things on the list I would like to see. I picked more jobs because ‘Get to Work’ was an expansion that I really loved. I loved the 3 jobs they gave us, they kept my sims very occupied. There are others, like those 3 I would like to see. Jobs like a teacher, Professor, Military Career, Architect, Model, Fashion Desingner, Actor, even a Mechanic, and Plumber. Some of the other things on the list that I liked are Pre-teens, smarter sims, more romance, more for kids and teens to do, and more interactions between them. Other things I would like to see are babies untied from the crib, cars, a teaching pack from pre-school through college. A farm pack with animals, Leisure vacation packs with motel/hotels, a beach venue with ocean swimming, ski mobiles, and sailing. An alpine vacation with skiing, tobogganing, snow mobile, etc. An amusement park vacation with rides and games, and a water park vacation with water rides. And of course a casino vacation with slots, cr*p table, 21, and a roulette wheel. Oh.... and I also want to see witches and Fairies.
I definitely want more fantasy options such as life states, dragons, unicorns, Elves Lord of the Rings style. I would love a historical pack with greek/Roman, medieval, Rococo, Tudor, pirates, Victorian. Not just clothes, but lots of furniture, clutter, etc.
I clicked University - but it is actually tied with Witches/ more supernaturals as well as University. I feel equal on the need FOR MY GAME of University and Witches in particular. But only being able to pick one choice left me unable to pick both these things even though they are equal needs for my game.

I miss the open world aspect of TS3 and previous iterations - it stinks that we're limited to the number of lots that already exist in a world. I hate that if I want to build another house in a world that's already full, I have to bulldoze an entire lot. I realize that probably won't happen in TS4, but I'd love to see it come back.
I'd also love to create worlds - decide what the climate is going to be, the terrain, how many public/private lots there will be, etc.
Bring back terrain modification!! I loved being able to put my sims like on a mountain or a hill or deep in a crater (unless I'm dumb and missing something, which is always possible). Being able to make the terrain your own would be amazing.
Can we get some more variety in the trees and plants department? I love landscaping and I'd love to see some more blossoming trees and smaller trees. I'd also love to get more flowers that aren't low to the ground or just bushes. More flower colors would be great too.
Also, why is it that in TS4 we have fire alarms, but we don't have a fire department? It was always such a relief for me in older Sims games that if there ever was a fire on a lot, that the Sims would be saved because the fire department was coming.
Bring back burglars!! The constant wondering of "Are my sims going to be robbed today?" was such a fun and exciting time during my old Sims games. Now sleeping is just boring.
I want more pool or beach-related items! Give us diving boards, beach balls, sand volleyball, lounge chairs, beach towels, etc.
Speaking of the fire department, bring back being a fireman as a job!! It was so fun getting to actually work as a fireman and put out fires and rescue sims in TS3.
Bring back vacation spots like in the original Sims (Ski Lodge was one -- can't remember the others at the moment) with those same activities - snowboarding, skateboarding, etc.
Let us customize items/clothes again!! It drives me nuts when I'm trying to decorate/design a room and none of the furniture really fits with the theme that I have made for that room or when I'm trying to design an outfit for one of my sims and then I end up having to go back and change the hair or the top because it doesn't come in a certain color.

Dates stink because they not only don't stay interested in each other or stay near by, but there's no interaction. I would think a date would have auto flirting together, HAND HOLDING as they walk, and other cute things. Impromptu dancing would be cute, snuggling in bed or at least spooning, SLOW DANCING should be a thing.

Yes I agree, Romance is one of those things in sims that is adorable to see so one of the most important parts to improve. I also think that they should be able to have crushes because when someone flirts with another they get romantic interest but I don't always feel they are both in love.
