Help with children death

I'm doing a ghost challenge and i have been trying with fire and drowning and it keeps glitching? Three different ways.
Child sim gets taken from household without the notice, but you can invite her over. It still shows her in the household as alive and well.
She disappears as if she never existed.
Death is successful, but you can't do anything with the urn except destroy it.
