Sims walking around naked

I've noticed in my game that my symptoms are walking around the house naked/pixelated after they take a shower/bath. this includes children adults and toddlers

Does it occur every time or randomly?
If you have cc/mods in your game, move your mods folder to the desktop. Then start the game and see if the issues are still there
