Yes, it works. Once I placed the sauna in an area in the basement that was completely under another room in the house it could be used normally. However if I place it in a room in the basement that has plain, uncovered ground above it, it does not work and cannot be used even though I confirmed that ceiling blocks are present.
Saunas currently don't work in basements at all. The issue was covered in LGRs review. Not sure if that's intentional or if it's just another case of build mode wonkiness. It's not a bug though, and you're not placing them wrong. Unless it gets patched or modded you won't be able to use them in basements.

so i tested i figured out the room for the sauna needs to be directly underneath an upper level structure that has a floor and a roof. the basement can extend pass the upper level but the room the sauna can not.

Same issue here. Can not even use the styled room sauna that came with the game in the basement. I hope they issue a fix for this soon.
hi @Drummzz7 it can work if you see postings above you will see if you place the room with the sauna directly under a top floor that has flooring and ceiling it will work. the room with the sauna needs to be completely below a top floor build, HTH.

My mind boggles at the calculation the game must be programmed to do to work out if a basement is a room or not!! I am trying to think myself into that programmers head and how he or she made that decision to do that test. I could understand if it was left as plain grass terrain above, but once you laid down patio tiles it should be a floor and ceiling. BUG!!!!! lol
Does anyone else get this issue? I made a sauna in the basement level of my home and Relax is greyed out because it says Must be indoors. Last time I checked the basement of my house was indoors?! And I have complete walls/doors/windows so it is fully enclosed.

Disappointing... hope this gets addressed soon as my current house is on pylons and thus has no ground floor under-which to build the basement sauna.
I did a bit of experimentation and it looks like just putting a floor over the basement "fixes" this, but it is inappropriate to my current lot design.

Sauna in question is in a basement, no foundation, no buildings over the basement where the sauna is.
