Maybe they mean like the way you put together outfits or "looks." Like, you still use the presets, but you decide how to put everything together and accessorize, then that's what you sell.
I'm going to guess that just means you get to pick the outfits (choose which shirt, bottoms, shoes, etc you want together), not actually design anything.

I mean, creating it from zero would require meshes and textures from scratch. Most of us wouldn't be able to do that.

We also had a comment that we will be able to knit in the game as well. Perhaps knit our own clothing and design it on the mannequin? i have no idea aha
You'll probably get to take your mannequin to the CAS and dress it like you do your sim. I don't expect too many additions. Otherwise.... I'd be my biggest customer buying all my newly designed clothes.
Now... if only I can find a good furniture shop... lol. Know of any?

That would be highly aggravating. I hope they add a way to customize what you make (Color's and designs, much like TS3)
Or worse, the store will only allow you to buy the clothing related objects (like the one we already have in game, exposed in the store on the trailer), like any other store.

I don't know, but if they add CASt with this expansion, that is one of the very few things that would bring be back to the game. I would actually buy that.

You will probably be able to choose among the existing clothing parts in cas and put them on a manequin and then sell those clothes, dunno.
To answer you're question, no unfortunately I doubt they'll let us create our own clothing to sell. That would be a very complicated feature for them to add and they can't even bring back create a style for the Sims 4. Plus it would give players a lot less reason to buy EA's stuff that they're bound to start selling via the Sims 4 store or something.
It would be a really cool feature if they could include it, but not having it in the game won't be that big of a deal. At least we'll get to run our own stores now, including clothing stores that were never featured in the Sims 3.
Nonetheless it's a cool feature. I like that idea alot. Could be a great "work around" townies wearing stupid ridiculous clothing as EP and theme specific stuff comes around.
In Sims 2 OFB, if your Sim bought an outfit as a "townie" in your store, or while you were out shopping, they have access. I don't know if anyone knows that. That's what many of us meant by "cohesive gameplay". So a Sim could buy their own clothes on their own free will and when you switched back to that household they'll own those clothes.
