Disappearing headstone

My Sims were at the graveyard placing Grandma and Grandpa's headstones. Grandma's was placed without a problem. But when Grandpa's was placed, it disappeared.

Is there any way of getting it back?

I had that happen once. I quit without saving, went back to the previous save, at the beginning of the same day, and tried again, placing the headstone in a different location. I figured it was probably one of those places where there is a defect, like when sims slip down into the ground at a certain spot. It might not have been, but the headstone stayed in place that time.

Unfortunately, I saved not long after that a couple of times. (I get Code 12 if I don't save about 3-4 times per Sim Day.)
