Yesterday before the patch I was playing this career and I got a guarantee diagnosis of triple threat. Said the patient needed surgery so I preformed it and in the end it told me I was wrong.. Anyone else having this issues??

Try seeing if the same thing occurs the next time that your sim goes to work. It may have been a random glitch that occurred.
