@imsimplyshanna When you ask "What's wrong?" does a thought bubble appear over the pet? My Sims have cats and one of the common things that shows is what looks like a Sim with a lampshade on their head.
I give the pet a squeaky toy and it seems to calm them at least temporarily.

The issue with pets being afraid after having a preventative shot has been fixed in a recent patch.
If you are still having issues, check that your game is uptodate,
Pets can also become uneasy for other reasons also.
So after doing the cheat. I hate using cheats but I had to do what I had to do to make little Bernie the Pembroke Welsh Corgi better again. I found out giving him the Wellness Treat will make animals perpetually afraid of "something". It is quiet literally nothing. But they are just "stuck" with the scared buff until you either delete the Sim or game or game data all together or use the cheat. Just thought I would let you know this. I am not sure if it is a bug but every single treat besides the Age Up or Age Down will give your little furry Sim a negative buff.
Also, Bernie has been to the vet and no negative side effect afterward. It was ALWAYS right after the Wellness Treat. The Wellness Treat didn't use to do this but now I am convinced that all of these treats besides the ones you buy for 5 simoleons, and the Age Up and Age Down are BAD for your pet.

My dog was afraid of something for about a week I tried everything then I gave him a scorching cinnamon vet treat and boom he became happy in less than a minute. It worked for me so try it and let me know. Good luck
I had the same issue with my dog so I gave her an ambrosia treat (which resurrects your dog) and it got rid of her fear and uncomfortable moodlet **you can get the ambrosia treat at the vet**
The exact same thing happened to my doggo, so after a few sim days of waiting for my dog to get over the fear, I decided to take him to the vet. I clicked the preventive shot and he is fine again!
I'm also having this kind of issue.. It looks as the cone of shame was stuck there.. it showed that it wasnt there but somehow it was... sadly this has not been fixed. I did put my cat for adoption and adopted him later, but the relationship didnt save... I lost a companion and a little kittys father.... i was able to get the file back but the kitty was still not born yet...

With the issue with the cone of shame, is it remaining there constantly ? Try having your sim and their pet travel to another lot and back, then see if that helps.
Repairing the game may help. To do that, go to games library in Origin, right click on the sims 4 picture and click repair
