When I tell my Sim to take a bath they will get in for a couple seconds and then get right out again. I changed the tub a couple times and the same thing always happens.

Other simmers are experiencing similar issues.
Some simmers have found that having the shower/bath combination item helps to prevent this happening. Try using this item and see if that helps.
Try repairing the game and see if that helps. To do that, right click on the sims 4 picture in Origin and click repair.
Try resetting your sims 4 folder and see if that helps. To do that, move your sims 4 folder ( that is in Documents -Electronic Arts - then the sims 4 folder is there ) to the desktop. Then start the game, and a new sims 4 folder will be generated. Start a new game and test to see if the issues are there. Exit the game and then add back the "options.ini" file and the "saves" , "screenshots" "tray" "custom music " and "mods" folders from the desktop sims 4 folder to the new sims 4 folder.
this is happening to me with one specific sim and cant get it to stop i have tried everything. she keeps getting out of the tub to "see what's happening?' theirs no windows or anything abnormal in the bathroom and her kids are find but she keeps doing this and its super annoying

resetSim (first name ) (second name )
(without the brackets ) and see if that helps.
I'm having this problem too, sometimes sims will stop bathing before they are done. Its like the desire to go use another object in the world overcomes their need to completely get hygienated.
Played Sims since the original. I can't think of any other game that has so thoroughly captivated my attention.

If you are finding that your sims are just getting in and out of the bath quickly , the issue can be caused due to a baby nearby. If this is happening, try moving the baby far away from the bathroom, and see if the issue remains,

Crinrict has written a blog post on the issues that simmers are having. It contains some workarounds that may help with the issues until there is a fix for the issues. It may help with your bathing issues.
