As Island Paradise is an EP, everything in it will be available everywhere in-game, no matter what world you play in. A Lifeguard profession requires a lifeguard chair on a beach lot, and can also be joined via newspaper or computer or going to the city hall to join.
If the beaches in your current world don't have lifeguard Chairs, you can add them in by going into Edit Town Mode (... button -> Edit Town), then selecting a beach lot and editing it (click the Build/buy Mode button in the lot description).

If you're playing a challenge to find all the uncharted islands, you need the Lifeguard career to unlock one of them. Other than that, yeah, it's a pretty boring career.
My socially awkward sims were hoping to perform standard resuscitation techniques on perfectly safe, non-drowning others at the beach as a form of preventative medicine. Doesn't seem to be working out too well...
I once has a sim who was a life guard in Bridgeport. It was nice, because there was only the one beach, but no one would show up to be saved until after 1 pm.
The guys were always trying their best to get this life guard to notices them. Later on they would also call her up for dates, that was what Daniel was attempting to do. (He was married at the time, so it was a no go for him.)
Just make sure you place the life guard chair on the beaches before you start. It does make a difference. Also make sure the beaches have full bathroom facilities, grill, and picnic benches. This way if your sim has certain needs, they can take care of them while on the job.
