Have you tired maybe reseating your graphics card?
(disconnecting from the motherboard and re connecting.) If you are unsure on how to do this then do not try it.
I think you have tried everything game related to fix this issue. Which leads me to think it maybe hardware or driver issues (or OS issues.)
Have you tried playing other games? other then the sims?
I did notice you had a few GameBar errors in your dxdiag. http://www.intowindows.com/how-to-disable-the-game-bar-in-windows-10/
GameBar has been known to cause issues.
@Gamingbross007 Unfortunately most of us that help in tech play on Nvidia/intel and why chester suggested you speak to @ChelleJo . She knows the ins and outs of AMD with this game.
@Gamingbross007 If your a heavy gamer or game at higher resolutions the 1080 ti would be a good choice. It's hard for us to help with AMD because most of us don't use AMD. If Chelljo or another AMD user comes on the board they can assist you with AMD and hopefully help fix your issue for now until you decide to invest in Nvidia. There is a lot more help on these boards for Nvidia users.
@chesterbigbird I have done this but it still doesn't work I also have graphics issues with Terraria not loading also surgeon simulator doesn't load also I now get bellow 30fps in euro/usa truck simultor even with graphics on low.

I would say this is hardware related then. You may have a failing card. Or a psu that's not dishing out enough power.
I NEED HELP!!! i have did the Origin turn off thingy, i have reset my computer, i have updated drivers and my video card. My video card is a GeForce GTX 750 Ti and running version 382.05. I lasted played sims4 in March and just tried to get back on......when i clicked to loaded it, Origin updated...... Please Help!!

Try uninstalling and reinstalling origin and see if that helps the issue.
Thanks, but I'm starting to think it's a card problem now since none of my games work...
Try checking to make sure that your graphics drivers are up to date.
If you recently installed new drivers, perhaps uninstall it and reinstall the drivers again.

I uninstalled drivers completely and re-installed and it worked yay ! Thanks !

Are you getting the same message? If you are , check to see that your graphics drivers are uptodate.
Try turning off origin in-game and see if that helps. To do that, go,to Game Library in Origin . Right click on The Sims 4 picture. Click on “Game Properties”. Then untick the box that enables origin in-game,

If you haven't already done this, repairing the game may help. To do that, right click on the sims 4 picture in Origin and click repair.
Try shutting down your computer completely again, Then restart the computer, log into Origin. and see if that helps.
If the issues remain, you could try uninstalling and reinstalling the sims 4 game. If you do, make a back up copy of the sims 4 folder first.
I am still getting error and i refuse to think my video card is outdated for sims4 yet i keep getting the error......i am legit about to cry...i really want to purchase the new pack but afraid i wont even be able to play it.

That error points toward a graphics driver issue.
Have you completely uninstalled your driver?

Uninstaller via add/remove programs in control panel then run the program above. Then install a fresh driver.
